Jakarta Dinsos Head, Premi Lasari said Jakarta residents who need physical assistance for disabilities could submit a letter to the Jakarta Dinsos or the Social Sub-dept in five regions.
Proposals or requests for assistance also need to be accompanied by a certificate of incapacity from the sub-district, photocopy of Family Card, photocopy of KTP, child's birth certificate (if the aid recipient is still a child), as well as a photo of the entire body of the prospective aid recipient.
